Appointments
We are always very happy to see you in our office, and will do our very best to see you as soon as possible while accommodating your schedule. Please call our main number, and our front office specialist will assist you with making the appointment. She will ask you a few questions to facilitate the scheduling process.

Patients will be seen by appointment, except for emergencies, which will be seen as soon as possible. If you need to change or cancel your appointment, we ask that you contact us at least 24 hours prior to your appointment time. However, do not hesitate to call even if it is the day of your appointment.

For patients who were referred to our office by another physician, please be sure we are aware of his or her name and address. If your insurance requires a referral from that physician, we ask that you request it prior to your visit. Unless otherwise indicated, it is customary for us to send your doctor a written report of our findings and treatment.


Before Your Visit
We want to make sure that we have everything we need to ensure that you receive the best service possible. Make sure you bring your current insurance card to your appointment, along with your referral number if necessary. Please inform your referring physician's office of your appointment, so that any pertinent medical records can be sent to us in advance of your appointment.

A complete and thorough medical history is necessary in order to accurately evaluate a patient and recommend further medical treatment. If you are a new patient, please complete the patient registration and medical history forms located on this website prior to your visit and bring them with you. This will save time for you at the office during your visit. Please be sure to read the Notice of Privacy Practices document prior to your visit.

Please plan on bringing with you to your appointment any pertinent medical records, x-rays, or lab work that we have not already received prior to your visit.


At Our Office
When you arrive at our office, one of our front office specialists will start the check in process. She will review your registration form and enter the information needed to establish your medical record and bill your insurance company.

Once your check in process is complete, one of our medical assistants will escort you to an examination room and ask pertinent questions related to your visit. Dr. Choudhry makes every effort to see you as close to your appointment time as possible, but it is difficult to judge how long each patient will require before his/her visit. In addition, Dr. Choudhry’s schedule is often fluid and changed by emergency surgeries or hospital consults. We ask that you be patient and understand that Dr. Choudhry will give you all the length of time needed for your visit.


Payments and Insurance
We require payment at the time of service for all office visits and procedures, including deductibles and co-payments for insured patients. We require a deposit of estimated patient responsibility, including deductible and co-insurance, before electively scheduled procedures or at the first visit following emergency surgery. We accept check, Mastercard, Visa, Discover, and American Express. We ask that you do not use cash if possible. We will assist you in filing an insurance claim. We participate in many medical health plans. Statements are mailed at least monthly and accounts not paid within 30 days are considered delinquent. Arrangements should be made if you cannot pay in full during that period.
